Search engines often parse text so that words that appear earlier in a sentence are weighted higher than words that appear near the end of a sentence. Page relevancy is calculated by the use of important keywords that describe the page content. A page about a specific topic should use a keyword related to that topic at the beginning of the <title> tag instead of using a site name or brand name, because those do not describe the contents on the page.

Recommendation

Place the targeted search term for the page at the front of the <title> tag. Do not use branding terms at the front of the <title> tag, because they are not directly related to the keyword for a given page.

These violations are from the Bing SEO Toolkit, and are the same violations that Bing finds when crawling the internet, it is a safe bet that Google would find much the same violations.
